For those who use Nero Cover Designer, use the Front [Rob Saunders] (Nero).ncd file.
If you don't have IrfanView, get it at http://irfanview.com, it's free.

For those who don't, I will lay out some simple instructions for the way that I do it.

Front Cover

1. Open the front cover image in IrfanView.
2. Press the 'L' key, or the 'R' key. This will rotate the image.
3. Press Shift+P. This will open the Print dialog.
4. Press the Print button. Now you get a second Print dialog.
5. In the Print Size area, select Custom, check off Aspect Ratio, and put 12 in the Width box.
6. Now press Print. This will print your front cover.
7. After printing is complete, cut the excess paper away, and fold at the middle dividing line.
8. Insert the cover in the front of your CD case.

Back Cover

1. Open the back cover image in IrfanView.
2. Press Shift+P. This will open the Print dialog.
3. Press the Print button. Now you get a second Print dialog.
4. In the Print Size area, select Custom, check off Aspect Ratio, and put 15 in the Width box.
5. Now press Print. This will print your back cover.
6. After printing is complete, cut the excess paper away, and fold at the white lines near edges.
7. Place the cover in the back of your CD case.

Disc Sticker

Good luck, I only ever printed one of these and I used some program called Neato CD Cover Maker. I just made it 'cus Protricity asked.
